INTERNET KIT

If you have internet access, prior to ordering your kit(s), go to the members log in page, and create an account.  If you need help creating an account, go here.  Once you have your Samoyed's unique "identifying code" and have completed the database entry, then pay online for your kits.  Your kits will be mailed to you and will include:

  1. five (5) cheek swabs

  2. instructions for how to collect the cheek swabs

  3. SCARF Transfer of DNA Ownership & Release form

  4. owners consent for researchers to contact them-optional form

  5. check list to complete your submission for the DNA Banking program

  6. shipping instructions

NON-INTERNET ACCESS KIT

 

If you are unable to complete the online database for Samoyeds, the following information will be mailed to you:

  1. print out of Samoyed Database entry form

  2. five (5) cheek swabs with "identification code" on each envelope

  3. instructions for how to collect the cheek swabs

  4. SCARF Transfer of DNA Ownership & Release form

  5. owners consent for researchers to contact them-optional form

  6. check list to complete your submission for the DNA Banking program

  7. shipping instructions

You will need to create your own "password", which should be less than 8 numbers and/or letters and is case sensitive. Should you want to modify any of your dog's information in the future, you will have to either call or mail the DNA Bank and provide the dogs "identification code" and your "password".  This is for the security and protection of the information you are submitting on your dogs.
