Upon receipt of this form and payment we will activate your username and password. You will be notified when your account becomes active by e-mail or regular mail. Once you are a member, you can upload and modify your animals information. The database will automatically generate links to you via e-mail, so that users of the database can directly interface with each other about their animals. You are responsible for the content and accuracy of the information that you provide. Please use the database responsibly.

None of your personal information will be shared as a result of becoming a member of this database.
