Welcome to The Poodle Club of Canada's Health Survey for Poodles.

This survey is CONFIDENTIAL. The Poodle Club of Canada and its members will receive statistical information only from this database.  The value of this information will be to track the health of Poodles in general and to allow  veterinary, genetics and medical researchers access to confidential information which will help them with their research.

You may
volunteer your dog's information for such research projects, and you may choose to fill in contact information to allow researchers to ask you for further information or assistance.

Since this survey is ongoing, you may use your dog's page to keep health records.

Every record is valuable; if we are to have a clear picture of the health of Poodles, we need all Poodles entered, whether they have developed a disease or not. One of the key pieces of information is longevity, so we ask you to return and fill in the date and cause of death if known.  If your dog changes ownership, we ask that you transfer the health record to the new owner, so that there is a complete record for the dog.

Information from Poodles in the past is also valuable; if you have records of now-gone Poodles, please fill them in as well.

There is a public
Poodle Health Registry. The PHR requires veterinary documentation for any dog registered. The public registry helps breeders to make wise and knowledgeable decisions about their breeding programme, and therefore leads to improved overall breed health. The PHR and the PCChs are both dedicated to improving the health of Poodles. Please consider entering your dog in both registries.

Thank you for taking the time to fill out this survey.
